Cal-AIM Housing Navigator
Overview:
Cal-AIM Housing Navigators play a vital role in fulfilling the agency’s mission by providing housing-related services to members authorized by Managed Care Plans (MCPs). They deliver transitional navigation services, housing deposits, tenancy support, and Community Supports housing services. This role collaborates with local Continuum of Care (CoC) systems, MCPs, and Enhanced Care Management (ECM) providers to ensure high-quality services.

Qualifications:
- Minimum: Bachelor’s degree or 3+ years of experience in case management, housing support services, or working with the homeless population.
- Preferred: Bachelor’s degree in a related field, knowledge of Coordinated Entry Systems (CES), and local housing authorities.
- Highly Desirable:
- Expertise in Motivational Interviewing, Trauma-Informed Care, Harm Reduction.
- Proven ability to establish partnerships with landlords and property managers.
- Strong organizational, communication, and problem-solving skills.
- Experience working with diverse communities and populations.
Responsibilities and Duties:
Housing Services
- Complete intake and assessments to evaluate clients’ housing needs.
- Develop and implement individualized housing support plans.
- Assist members in securing affordable housing by networking with landlords and navigating the rental market.
- Support clients in applying for subsidized housing, including completing applications and compiling documents.
- Conduct pre-inspections of housing units for compliance with Housing Quality Standards (HQS).
- Provide mediation services for landlord-tenant disputes to help maintain housing stability.
- Educate clients on tenant rights and responsibilities.
- Facilitate access to resources like security deposit assistance, household goods, and utility arrangements.
Ongoing Support and Case Management
- Conduct regular home visits to ensure habitability and address health and safety concerns.
- Monitor housing progress through weekly meetings with clients.
- Identify and address potential behaviors that may jeopardize housing stability, such as late payments or hoarding.
- Connect clients to community resources to prevent eviction and support housing retention.
Collaboration and Advocacy
- Work closely with MCPs, CoC systems, and community partners to coordinate care.
- Engage in outreach to isolated clients and promote recovery through community programs.
- Assist with benefits advocacy, including obtaining identification and applying for SSI.
Documentation and Compliance
- Maintain detailed records in HMIS, Salesforce, and other required systems.
- Ensure compliance with HUD regulations and program standards.
- Prepare service notes and reports accurately and on time.
Other Responsibilities
- Represent the Cal-AIM program in community presentations.
- Attend staff meetings to contribute to program development.
- Continuously update knowledge of best practices and local resources.
